My Premise Health is a secure online and mobile tool that connects you with your healthcare experience by providing convenient virtual visits, access to your providers, health records, visit history, test results, and more.

To Sign Up:

  1. Visit mypremisehealth.com or download the mobile app
    from the App Store or Get it on Google Play

  2. Click on “SIGN UP NOW”

  3. Sign up by entering your full name, date of birth and last four digits of your social security number (SSN).

+ Is there an app for My Premise Health?

Yes. You can download the My Premise Health app from the App Store or Get it on Google Play.


+ What can employees do from the mobile app? Can the mobile app send notifications?

All functionality available online will also be available on the mobile app. The mobile app sends notifications only for transactional notices as related to My Premise Health and only if the employee has accepted text messages as a form of communication.


+ Who do I contact for My Premise Health technical support?

Email myphsupport@premisehealth.com and provide a phone number for a return call. Available Monday - Friday, 6 a.m. CST - 7 p.m. CST.

There is also a “Contact Us” link available at the bottom of the page once you have logged into the My Premise Health portal. The link directs you to a form that can be completed and sent to the support team. Premise Health will respond within two business days.


+ How do I reset my password or unlock my account?

You may use the “Forgot Password” link on the login page to reset your password. If your account is locked, you may email mypremisehealthsupport@premisehealth.com.


+ Will my past health information be visible to my healthcare provider?

Yes. Your historical patient data will be accessible by your healthcare provider and Wellness Center team in order to ensure continuity of care.


+ Do I have a right to and how do I obtain a copy of my health records from Premise Health?

Yes. To obtain a copy of your individual patient record from Premise Health, you will be asked to complete an Authorization for Release of Medical Information Form. The purpose will state “For Individual’s Use” if no other purpose is given.


+ Does NFCU or Health Services have admin rights to the new patient portal (i.e. can others access my account)?

No. All health information is kept secure. Only you and your healthcare provider(s) are able to view your account.


+ Are there blackout times for website updates?

There are no blackout times for patient portal.
